Check in can be done in office on an iPad or on the portal prior to arriving.To expedite the process, reduce your wait time and maintain social distancing, please complete your check-in process on the portal prior to your appointment.
TO ENTER THE PORTAL FOR THE FIRST TIME YOU MUST USE THE LINK PROVIDED IN THE E-MAIL WE SENT YOU TO CREATE A USER NAME AND PASSWORD.
- First verify your email: You should have received an email from [email protected] to sign up for our patient portal.
- Click on link in the email you received to create a username & password
- Answer all intake questions
- Sign the Consent forms (Under "Appointments" at the TOP RIGHT).
- If needed, click here for detailed instructions.
- Masks are optional. Please feel free and comfortable if you wish to wear your mask in the office.
- Please call and reschedule your appointmentIf if you have any concerning symptoms such as fever, coughing, sneezing, diarrhea, vomiting OR known exposure to COVID-19.
In the event that you need to cancel or reschedule your appointment, please call 24 hours prior or a no-show fee of $50 will apply.

Medical Records:
We prefer records being sent to our office coming by e-mail or sent by mail, we do not accept records sent via fax. Please fill out appropriate form for medical records release and reach out to us for appropriate e-mail address or mail paper copies of records directly the office.
